Humulin or Lantus Which Insulin For Your Child?

Humulin or Lantus? When my daughter, who was 8, was first diagnosed the Children's Hospital that was treating her put her on an insulin program of short acting Humalog NPH and long acting Humalin N. You should have seen me that first day of training after a long night in the emergency room where she was diagnosed. I was a raving lunatic to begin with so my brain was mostly mush at that point. Add to that everything began with H. Mix this H with that H but first charge this H with air, then draw this H first followed by that H but make sure you roll this H between your palms and for crying-out-loud don't shake that H. Anyway I guess osmosis worked and all of that information finally seeped in. We were officially diabetic now.

For those of you who may not be familiar with it, Humalog and Humalin (short and long acting insulins) are mixed in the same syringe and injected 3 times a day usually before meals. Your child is allowed a certain amount of carbohydrates for each meal that the doctor figures out based on age, weight, etc., and that's your number. My daughter's was 65 carbs max per meal and 35 max per snack. However your child hits their magic number that's it. Whether it's 65 slices of ham or half a pop-tart. Plus she had to eat 6 times a day at certain times everyday.

Well the time came when she decided she wanted a little more freedom of choice and asked me about trying Lantus. My first thought was if it's not broke don't fix it. But I can have a cupcake whenever I want so I decided my opinion was secondary. So we looked into it. Lantus is a long lasting insulin. One shot at night and she had a 24 hour basal dose of insulin. One shot I'm thinking to myself that's great. But wait, you also have to take a shot of short acting insulin (a bolus) everytime you eat. 10 meals 10 shots, 3 meals, 3 shots. Even I could figure that out. You base the amount of short acting insulin on the number of carbs you are about to eat. Talk about freedom! Compared to what she had been doing it was like being re-born. she didn't care that it would mean more shots. All she heard was she could eat what she wanted when she wanted. Birthday cake, pizza parties, popcorn at the movies, boy she was ready to go. The first thing she wanted to have was a Blizzard from Dairy Queen. I looked at the chart on the wall and it read 125 carbohydrates. That was half a days carbs in a paper cup! After a couple of days of gluttony things got back to normal pretty quickly.

Remember that freedom I mentioned, it came at a price. This insulin regime is a lot of shots. Add those to the required blood tests and your talking about a lot of holes per day in a little girl. One more thing, her nighttime Lantus shot was 19 units. that's a lot of insulin all at once. In her case it went in like battery acid. These shots are no fun. However, after several months on the Lantus regime her life and the rest of my family's lives are pretty close to pre-diagnosis normal. All in all it was the right thing to do. If your doctor is suggesting Lantus or your child is asking about it, consider all of the above. We're glad we did it.

How Refined Carbohydrates Can Harm You

I advise my diabetic patients and people who are trying to lose weight to avoid all foods made with refined carbohydrates because they cause a high rise in blood sugar. Even if you are not diabetic (yet) and are not concerned about your weight (yet), you should understand why the large amounts of refined carbohydrates in the typical Western diet can harm you.

Refined sugars and refined grain products (anything made from flour, milled corn or white rice) have had vitamins, minerals and other nutrients removed in processing. Some but not all of these nutrients may be added back in enriched flours or fortified foods.

From 1600 to 1930, more North Americans died from the vitamin deficiency diseases, beriberi and pellagra, than from any other cause. These diseases disappeared when governments legislated that all flour had to have three vitamins, thiamin, niacin and riboflavin left in or added back. Over the last 70 years, the incidence of heart attacks in the United States had been increasing until Congress legislated that folic acid must be left in or added back to flour; now the heart attack rate is decreasing. However, diabetes and obesity continue to increase at alarming rates in all age groups.

Where carbohydrates are found in plants, the B vitamins are also found. Carbohydrates are combinations of sugars, either as single sugars or chains of sugars from two to millions. When you eat carbohydrates, enzymes in your intestines break them down into single sugars and only single sugars can pass from your intestines into your bloodstream, where they can be used for energy, stored as sugar in your liver or muscles or be converted to fat. Many different chemical reactions then break down sugar one step at a time to release energy. Each reaction must be started by an individual chemical called an enzyme and the B vitamins are parts of these enzymes that start the reactions that break sugar into energy.

If any of the B vitamins are not available, the conversion of carbohydrates to energy is blocked. Instead, the carbohydrates are converted to fat which:

* raise blood levels of triglycerides;
* uses up the good HDL cholesterol, which lowers blood levels of HDL and increases risk for heart attacks;
* is stored in fat cells primarily in your abdomen;
* helps form plaques in arteries, which makes them stiff and raises blood pressure; and
* blocks insulin receptors on cells so you cannot respond adequately to insulin. This causes you to produce more insulin, which makes you hungrier, makes you store more fat, and leads to diabetes in susceptible people.

When you eat carbohydrates that have been separated from the B vitamins, minerals and perhaps other nutrients which have not yet been identified, you increase your risk for diabetes, obesity, heart attacks and high blood pressure. We do not have enough dependable research to know if taking the B vitamins separately (in other foods or supplements) is as healthful as eating the B vitamins as they come in nature, paired with the carbohydrates in whole grains, beans and other seeds, vegetables and fruits. Your best bet is to eat your carbohydrates as nature packages them, and limit or avoid those that have been refined into sugars, flour and milled grain products.

When people get lung cancer, people say, 'Did you smoke?' Well, the problem is about 10 percent of men and over 20 percent of women with lung cancer never lit a cigar. Why among that group, women are two to three times more likely than men to get the disease, doctors don't know yet. Hormones, diet, air pollution and especially second-hand smoke, all are believed to be important factors.

Secondhand smoke (SHS) - also referred to as involuntary smoking, environmental tobacco smoke, or passive smoking - is a mixture of the smoke given off by the burning end of a cigarette, pipe or cigar and the smoke exhaled from the lungs of smokers. Tobacco smoke lingers in the air hours after cigarettes have been extinguished and it is involuntarily inhaled by nonsmokers.

Secondhand smoke is a known cause of lung cancer, heart disease, chronic lung ailments such as bronchitis and asthma (particularly in children), and low birth-weight births. Even a half hour of secondhand smoke exposure causes heart damage similar to that of habitual smokers. Nonsmokers' heart arteries showed a reduced ability to dilate, diminishing the ability of the heart to get life-giving blood.

In addition, the same half hour of secondhand smoke exposure activates blood platelets, which can initiate the process of atherosclerosis (blockage of the heart's arteries) that leads to heart attacks. Not surprisingly, when faced with the variety and range of products on the market today, the average consumer can find this selection process a little more overwhelming than ordering a cup of coffee.

Tanning products or sunscreen agents are not only available in multiple forms including sprays, lotions, sticks, gels etc.: they can also be targeted to different activities, age ranges and skin types. They are produced by both smaller (less familiar) manufacturers, as well as by the more popular brand name manufacturers such as ?Banana Boat', 'ChapStick', 'Clarins', ?Clinique?, ?Coppertone?, ?Estee Lauder?, ?Johnson?s?, ?Neutrogena?, ?Oil of Olay?, and ?Vaseline?.

In response to the demands of consumers, manufacturers continue to provide an ever increasing number of products for you to choose from, with many targeted to specific markets. However, the variety of choices doesn?t mean it makes the selection process easy.

So where on earth do you start?

Besides your skin type, here are some simple tips to consider when making your selection from the variety of tanning and sunscreen products available:

a) Chosen Activity: ? Being outdoors is not the only element to take into consideration when factoring in your amount of exposure to the sun. You also need to note the intensity with regards to climate, the season, location and time of day.

For example, just because you may be hiking in a cool mountain region or snow skiing, doesn?t mean you should throw all caution aside. Wind and UV rays still reach you, so protection would be wise. Consider also any exposure to surfaces with reflective qualities (i.e. snow, water, sand, etc.), as these can intensify UV ray harm. Activities that make you work up a sweat or those involving water (e.g. basketball, jogging, sun bathing on a hot beach, biking, water skiing, jet skiing, parasailing, outdoor work and exercise etc.), have the potential to wash off your sunscreen, and therefore need to be addressed also.

Choose a sunscreen product for these activities with both UVA and UVB protection and an SPF of 15 or higher, when possible. As for water / sweat activities, choose a product that offers a waterproof or water-resistant agent.

Tip: Where appropriate, you should also consider dressing to cover up as much skin as possible (e.g. wearing long sleeves, a scarf or hat, slacks or jogging pants).

b) Age: ? There are only a few words of caution about age. First of all, health care providers do NOT recommend using alcohol-based sunscreen products on children. And they do NOT recommend using sunscreen agents of any type on babies under the age of 6 months. For children older than 6 months, a lotion is the preferred form of sunscreen, over a spray, for example. And the SPF should be a minimum of 15.

c) Use on Your Body: ? Your choice of tanning product also depends upon where you are going to apply the lotion, gel, spray or other form. And it also depends upon the health of the area of the body.

For example, there are lip balms with sunscreen (solid or gel-based ones), which not only help the lips, but can also touch up spots in a hurry. Also if you?re trying to apply protection to fingers, ears, nose and other small areas, a physical sunscreen agent, lotion or gel may be easier than a spray. If you have dry skin, you might want to try a product in lotion or cream form. For oily skin, gel-based or alcohol forms of sunscreen might be better choices. N.B: A word of caution: Do not use alcohol-based products with inflamed skin or eczematous.

Keeping the above considerations in mind, should give you a great starting point from which to choose your tanning product or sunscreen agent.

In addition, a recommendation from a friend for a particular product, (assuming they have similar skin type and circumstances), - or even a particular brand name (remembering that the more popular manufacturers produce products catering to different skin types, activities etc.), can sometimes also be helpful in narrowing down your choices.

Ultimately the choice should be based upon your individual situation, your skin type, and your personal preferences. What works for one person may not be the most ideal for another.

Once you've done some basic research and have found a particular product you feel is suitable, then why not just start with that product. If you find it doesn?t offer you the results you expected, then simply be prepared to do a little more research, look around further and modify your choices accordingly. While there may be some trial and error involved initially, by becoming more familiar with your preferences and the types of products available, you're sure to find the product that?s right for you.

Several of the most effective poses are the standing positions. They provide superior stretching of the muscles and they have recognizable effects on the speed and efficiency of the nervous system. Most standing poses manage to enhance the asanas and they offer you an increased opportunity of mastering equilibrium, both physical and mental. In the following paragraphs we will take a look at two of the most significant standing poses, the Mountain pose and the Triangle pose.

The mountain pose (known as the Tadasana) got its name from a number of defining properties that share the symbols of the mountain. The next significant standing pose is the Triangle pose, or the Trikonasana. This comparatively easy pose has a satisfactory stretching effect on the spine, giving it a good lateral motion that complements the stretching of alternate forward poses. The straightness of the knees is essential while performing this pose, as this will allow your movements to be fluent and to stretch all the targeted muscles and organs. Bending to the left and right needs to be done gradually and fluently. This is one of the yoga poses that is good for providing the foundation for the next levels of postures, which are more advanced and harder to perform. The stimulation of the spinal nerves is also useful and it improves complete body flexibility.

In order to enjoy the full advantage of the triangle pose you have to position your body properly. Your feet have to be spread apart while you are pointing to your toes. Try to alternate the pointing motion from your left foot to your right one while keeping a constant rhythm and perfect balance. After you extend your arms parallel to the floor you should breathe in deeply, allowing the energy to reinforce your movements. While exhaling aim to execute a slight bend to either left or right while sliding your hand down your foot. This motion requires a lot of flexibility in the lower back muscles area, so a good warm up session is completely essential before attempting the triangle. 5 Ways to Prevent Heat Exhaustion in the Workplace

Individuals who work in industrial workplaces suffer from heat exhaustion on a daily basis. Some work environments can get as high as 45 degrees which can potentially become very dangerous for workers. Heat exhaustion is a condition which is caused by over exposure to heat which results in the deterioration of bodily fluids.

  • Drink Lots of Water - Dehydration is a key factor in heat exhaustion. The hotter you are, the faster your body absorbs liquids and the more water you will need in order to function properly. Always have water on hand and try to keep yourself as hydrated as possible at all times.

  • Work in Intervals - Working an entire shift without a break is sure to put you at high risk of heat exhaustion. Take a break every hour or so to ensure that you get a breath of fresh air. Separate yourself from the overly hot work environment because being in those conditions for 8 hours straight is unsafe.

  • Take the Night Shift - Depending on your situation, this factor may or may not be possible. The hottest times throughout the day are between 11 in the morning and 4 o'clock in the afternoon. That said, (if it is possible) I would highly recommend working during the night when temperatures are considerably lower.

  • Rest - Make sure to get a good night sleep before a long day in an industrial workplace. Rest will energize your body and mind for the day and will make you less likely to become weak and suffer from heat exhaustion. At least 7-8 hours sleep is recommended per night.

  • Eat - Like resting, eating will also provide you with the energy your body requires to fight off heat exhaustion. Make sure when you take your breaks, you also have a small snack which will keep you fueled for a couple more hours.

    Following the guidelines listed above will decrease your likeliness of suffering from heat exhaustion on the job. If you do find yourself experiencing the symptoms of heat exhaustion, leave work for the day. Heat exhaustion is your body's way of letting you know that it needs to cool down and rest. Ignoring the symptoms can put you and your body in severe amounts of danger.

    Shift Work Sleep Disorder Are You Affected?

    Shift workers really do get a raw deal when it comes to getting enough sleep. They have to try and sleep when the rest of the world is waking up and their body clocks have a hard time adjusting.

    Short rotation shift workers are the worst affected. They have to change their sleeping patterns every few days. No sooner does the body's rhythm adapt than it's time to change it again.

    I remember many years back I was boarding with a friend who was a shift worker. I used to envy him as I passed him on the stairs heading towards a comfy bed, while I was starting a new working day, tired from being awake some of the night (I suffered from insomnia back then)

    Now when I think back, I realize how wrong I was. My friend was sleep deprived even when he managed to get 6 hours - which wasn't often. The sleep he was getting during the day just wasn't as restful. It would be like having permanent jet lag.

    Shift work and health

    Sleep deprived workers are less productive and more prone to accidents at work or while driving, they may suffer with indigestion a lot as the digestive system becomes sluggish at night. The effects of shift work becomes even harder to cope with as we get older, maybe because are bodies are less resilient.

    Lack of restful sleep can also depress the immune system leaving the shift worker prone to more colds, flu and other health problems. And then there the ever present social hassles. The shift worker has to sleep while their friends and family are awake.

    So what's the best way of getting a good day's sleep when you're a shift worker? The main problem to overcome is the body's natural circadian rhythm. It is designed to make us sleepy as darkness falls and awake as daylight floods our bedrooms. So all our systems are trying to slow down while we're trying to get going.

    Darkness stimulates the release of melatonin by the pineal gland. The production slows down as it becomes light. Melatonin encourages sleep and the lack of it can cause sleeplessness. This is known as the shift work sleep disorder

    The following can help.

    Invest in heavy blinds and curtains to keep light out of the bedroom

    Buy a sleep mask and some ear plugs Turn off the phone or have it somewhere you won't be disturbed

    Make sure your family and friends know not to disturb you during your sleep time Try melatonin half an hour before going to bed. This can help to reset you body rhythms. Start with 1 mg

    Bright light therapy can also help reset the body's clock and regulate melatonin levels. Seek medical advice first.

    The following are not recommended!

    Drinking alcohol to help you sleep - have a drink by all means, just don't expect it to improve your sleep.

    Sleeping pills - They're ok for a very temporary fix.

    So try the above sleep tips and see if they help your problem. It may well be worth having a chat to your employer about a rest break a on the job. After all it will increase your productivity so you?ll both benefit! ? ?

    Depression: Can Dolphins Relieve Depression Symptoms?

    There are various treatments for depression but this one can be not only original, but also effective. Swimming with dolphins can be an effective therapy for depression according to a research carried out in Honduras. It was known that these animals had healing properties for other diseases but not alleviating depression.

    Under support of biophilia theory, which shows how human health and wellbeing are dependent on our relationships with the natural environment, the research showed that swimming with dolphins is an effective treatment for mild to moderate depression.

    According to the British Medical Journal a randomised controlled study of animal facilitated therapy with dolphins in the treatment of depression.

    The study was performed in Honduras with 30 patients diagnosed with mild or moderate depression. 50 per cent of them were assigned to the experimental group and the other 50 per cent to the control group.

    Patients of the experimental group swam and snorkelled in the water with dolphins for one hour a day during a two-week period. Meanwhile, partients of the control group exercised the same water activities, but without dolphins, in order to control the influence of water and the natural setting.

    Antidepressant drugs or psychotherapy were discontinued for all patients at least four weeks before the research and they were also not allowed to take drugs during the study. The average severity of the depressive symptoms was more reduced in the experimental group than in the control group.

    The experts that led the study stated that effects caused by the animals were significantly greater than those of just the natural setting. They also explained that the emotions raised by the interaction with dolphins may explain the mammals? healing properties.

    Participants from both groups reported lasting improvement and did not require treatment even three months after the study. Which This suggests that in patients with mild or moderate depression, using drugs or conventional psychotherapy may not be necessary when biophilic treatment with animals is used, they conclude.

    My Comeback To The Gym: Why Didn't I Lose My Strength?

    Yesterday, I made comeback to the gym after a 16 day complete break (no weights, cardio, nothing!). Also, in those 2 weeks, I wasnt exactly eating very healthy (it's the holiday season - and I'm only human!). I also stopped Tribulus and Creatine supplementation.

    Normally, when you take such a break from the gym, you would realise that your strength levels would drop and you wouldnt be able to work with the weights you normally handle when you return. This, of course, is absolutely normal - and your strength levels would return in a few days or up to a week. This happened to me every time I took a break, or was forced to take a long break from my regular training (sometimes due to illness/work). In fact, this is normal for everyone else I know who trains.

    Anyway, the weirdest thing happened yesterday. Was planning on making a 'slow-comeback' (easing into my regular routine - starting with much lighter weights so that I dont get extremely sore the next day), and I was surprised to learn that my strength levels were at almost 100% as it previously was. No way!

    Out of curiosity - I then grabbed a barbell and loaded my usual weights for bicep barbell curls (2 x 22lbs (10kg) plates) and was able to successfully complete a set of 8 reps! No way!

    The Organ Shortage: Are Your Wishes Known?

    The development of organ transplantation is one of the greatest accomplishments of modern medicine. Unfortunately, supply is not meeting demand. Thousands of people die every year waiting for a life saving organ. According to NORS, the Nebraska Organ Recovery System, ?every 16 minutes another name is added to the list of Americans waiting for life saving transplants.? As of June 4, 2004, the Organ Procurement and Transplant Network reported 85,707 people on the organ waiting list, 408 of those are people in Nebraska. 206 Nebraskans are in need of a kidney, 130 need a liver, 15 are awaiting a pancreas, 25 need both a kidney and a pancreas, and 14 of those are waiting for a heart.

    One organ donor has the potential to save seven lives. One tissue donor can help more than 60 people with their donation. In theory, there could be hundreds of thousands of organs available each year. Unfortunately, according to the Organ Procurement and Transplantation Network, there were only 25,459 organ transplants performed last year.

    There are several factors behind the organ shortage. Only a small percentage of people who might serve as organ donors actually do so. The reason for this is that only a small amount of people die in circumstances that make organ donation possible. Other factors include: reluctance to become an organ donor, and family refusal to donate organs of a deceased loved one. Because such a small number of potential donors become actual donors, the question arises as to what kind of policies need to be instituted in order to increase organ availability.

    The issue of organ donation also raises several ethical issues. Some feel that procuring organs without family consent shows a lack of respect for the grieving family. Others feel it is unethical to disregard the wishes of the deceased. Despite this ethical debate, the Nebraska legislature has passed LB138, known as First Person Consent, in an effort to increase organ donation. This law states that a legal document indicating an individual?s desire to become a donor upon their death will eliminate the need for next of kin consent. This law ensures a person?s wishes to be an organ donor will be carried out.

    Nebraska residents can register online to become an organ and tissue donor. The Donor Registry will be considered legal declaration of an individual?s desire to donate in the event they should die. Once the donor registers online, he or she will receive a notification card in the mail. The donor must sign it and return it to NORS to complete his or her registry. The family of the donor will be notified at the time of death, not for consent of the donation of organs, but instead to inform the family of the decision to be a donor. The family will be asked to provide information about social and medical history as part of the donation process.

    Besides being opposed by some, there is another downside to First Person Consent. The Donor Registry is very complex and costly. Registries must make certain that names of registered donors are available immediately upon registration to organ procurement organizations. These names must be able to be accessed 24 hours a day, 7 days a week.

    The positive side of First Person Consent is that it shifts the burden of decision-making during a very stressful time, away from the family. The decrease in refusal by donor families will increase the supply of donated organs.

    With First Person Consent, a donor?s wishes cannot be overruled by his or her family if that person has registered his/her desires with the Donor Registry. The donor?s wishes will take precedence. Organ donation also brings up legal issues with autonomy rights. Autonomy rights are designed to protect the interest of individuals, allowing them to pursue interests that they find valuable. Prior to First Person Consent, the question arose as to whose rights and autonomy were being respected, those of the donor or those of the next of kin.

    One of the biggest cited reasons for ignoring an organ donor card is fear of being sued. The Uniform Anatomical Gift Act, established in 1968, and adopted in all 50 states in some form or another, recognizes the right of the patient to donate over the family?s right to refuse. Therefore, there is little reason to fear liability for acting on a valid donor card.

    What Causes Hyperhydrosis?

    We naturally perspire, all through the day, to help our body regulate its temperature. In fact sweating is necessary for the healthy physiological functioning of our bodies. It is a mechanism in which fluid containing salts is released, by which the body is able to dispel heat produced by exercise and hot environments.

    However, in some people, this natural perspiration is excessive, much beyond what is vital to cool the body. This condition is called hyperhidrosis. Although we do not know the precise cause for hyperhidrosis, it is commonly assumed to be a result of the over activity of the hypothalamus, the centre that controls the body temperature in the brain, or the sympathetic nerves, which run along the vertebra of the spine in the chest cavity.

    Did you know that there are over five million sweat glands, distributed all over our body? Or that over two thirds of them, roughly 700 glands per square centimeter, are found on our palms alone? These are controlled and maintained by the sympathetic nervous system, which is an independent part of the nervous system, and over which we cannot exercise any control. This chain of nerves controls the aprocrine and the eccrine glands, which are responsible for perspiration of the entire body. Over stimulation of these nerves manifests itself as excessive sweating. The part of the body that is affected depends on which part of the nerves is over active.

    Hyperhidrosis maybe generalized or focal. In focal hyperhidrosis, most often, the hands (called Palmer Hyperhidrosis) and the feet (called Planter Hyperhidrosis) are found to be affected. In some people however, it may be localized to the face and the armpit (axillae) area. Small changes like an increase in air temperature, exercise, fever, anxiety or even spicy food, can set off an attack of sweating. It usually reduces during the night and may completely disappear while the person is asleep. Focal hyperhidrosis usually manifests itself in childhood or adolescence. General hyperhidrosis, in which sweating occurs all over the body, can be caused by diabetes, chronic infectious diseases, hyperthyroidism, menopause and even obesity.

    Whichever area is affected, hyperhidrosis is usually psychologically distressing. It is an embarrassing disorder and can hamper many daily activities. Even simple tasks like shaking hands or writing neatly can become a problem. Clothing may have to be changed several times a day. It can also lead to social anxiety disorder. However, patients affected by hyperhidrosis are said to have a physiological condition and are rarely diagnosed with psychiatric disorders.

    The unfortunate part is that hyperhidrosis is not a temporary condition. Patients usually suffer from it for many years, sometimes right from adolescence, before seeking help or treatment.
